At North Carolina Community Family Medicine, each one of us is committed to providing care of the highest quality to our patients within an environment that is compassionate and ethical. We have been serving Chatham, Lee, and their surrounding counties since 1982. The practice has been affiliated with Central Carolina Hospital since 1987. North Carolina Community Family Medicine, has grown to seven providers including four primary care physicians and two family nurse practitioners, and one Physician Assistant. Services by our providers range from infancy through pediatrics, adult medicine, women’s health, occupational medicine, geriatric and hospice care.
Community Family Medicine has been awarded the status of Level III / Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance ( NCQA).
PCMH is awarded to physicians/practices who demonstrate excellence in standard of care to their patients. The purpose is to establish partnerships between health care providers and patients through the use of registries, information technology, health information exchange, and other means to assure that patients are receiving quality care.

- Same day service for most urgent health problems
Our providers have admitting privileges at Central Carolina Hospital. Our providers maintain professional relationships with UNC and affiliates, WakeMed, Durham Regional, Duke, FirstHealth, and other medical facilities. Community Family Medicine strives to meet the needs and preferences for continued care for our patients.

James B. Holt, M.D.
Family Medicine, Obstetrics
Dr. Holt specializes in geriatric medicine. With over 33 years of experience, he treats patients of all ages with both acute and chronic illnesses. He has been the Medical Director of UNC Hospice for over 30 years and serves on the Foundation Board of Central Carolina Community College. He is an avid bird watcher on the side.

Erik S. Butler, D.O.
Dr. Butler specializes in Family Medicine and cares for patients of all ages. He practices preventative care while also treating patients with acute and chronic medical conditions. Dr. Butler completed residency at Duke University in 2007 where he was Chief Resident and Resident of the Year in Family Medicine. He is board certified and is a member of the American Academy of Family Physicians, and the North Carolina Academy of Family Physicians.

Dawn Erikson, Family Nurse Practitioner
Dawn Erikson is a Nurse Practitioner who specializes in Family Medicine. She treats patients of diverse ages and socio-economic backgrounds in a family based primary care setting. She takes a relational approach to practice which has resulted in trusting and enduring bonds with patients, and has an extensive knowledge in herbal medicine and alternative modalities of healing. Dawn has served as a Nurse Practitioner for a variety of different practices ranging from Family Medicine, Women’s Health, and Surgical to name a few.

Daphne J. Cates, M.D.
Dr. Cates specializes in Family Medicine with a special interest in pediatrics and women’s health. She treats patients of all ages with both acute and chronic medical illnesses. Dr. Cates has served as the vice chief of medicine at the Maria Parham Medical Center and as the Chief Resident in the University of North Carolina Family Medicine Residency Program. She is a member of the American Medical Association, the American Academy of Family Physicians, and the North Carolina Academy of Family Physicians.

Cynthia Julich, Family Nurse Practitioner
Cynthia Julich is a Nurse Practitioner who specializes in Family Medicine. She treats patients of all ages with both acute and chronic medical illnesses. Cynthia is interested in nutrition related to sports, weight loss, diabetes, hypertension and cardiovascular disease. She enjoys working in Sports Medicine and Behavioral Medicine related to parenting learning and surviving life in special circumstances.

Leah B. Gruber, Physician Assistant
Leah Gruber is a board certified physician assistant currently practicing in family medicine. She earned both her Bachelor of Science in Medical Studies and Master of Science in Physician Assistant Studies from DeSales University in Center Valley, PA. She has a certification in Advanced Cardiac life support and is a member of the American Academy of Physician Assistants. Leah treats patients of all ages with both acute and chronic medical illnesses. She has particular interests in cardiovascular disease, diabetes, preventative medicine and pediatrics.

Insurance and Billing
Our office participates in most of the insurance plans offered in this region of North Carolina. Our insurance department files electronically with most carriers. If we are not a participating provider with your insurance, we will file your claim to your insurance for you out of courtesy. Insurance cards must be presented at each office visit to assure accuracy in billing your insurance carrier. The insurance staff has over 25 years of combined experience in working with commercial and managed care companies. They are well equipped for working with our patients and their insurance needs.
Copays, deductibles and coinsurances are due at time of service. Self-pay patients must pay at least $75.00 before being seen, and if the balance is paid in full when checking out, you will receive a 30% discount toward your office visit. We have a return check fee of $30.00 that will be added to your account for any returned checks. We accept Cash, Checks, Money Orders, Visa, Mastercard, and Discover.
